In order to save time, the initial evaluation forms are available below. You may print the forms, fill them out and bring them with you for your first visit. Not all of the forms are available on-line, you might need to fill out an additional form when you arrive. We do ask you to arrive 15 min. early for your first appointment. If you are bringing your completed paperwork with you, please plan to arrive at least 5 min. early.

You may have some questions for your first visit. Here are a few of our common ones.
Q What should I wear to my appointments?
A Something comfortable that you can move in.
Q How long are the sessions?
A All sessions, including the initial evaluation are 50-55 min.
Q What should I bring to my first appointment?
A Please bring your insurance card, doctor’s referral and your driver’s license. If you are under the age of 18 we do require you to bring your parent or guardian (at least for the first visit).
Q Why is there so much paperwork?
A We prefer to collect a full medical history from every patient. Past injuries and illnesses can play a role in current issues or pain you may be experiencing. The more information we have the better we can help resolve your issue.
Q How much treatment will I receive for my initial evaluation?
A This is depending on how extensive your medical history and current condition. We do try to get in at least 30 minutes of treatment on the initial visit. Our main goal for the initial evaluation is to do a full and complete evaluation and create an extensive treatment plan. This can take a little time to begin to pinpoint where an issue is stemming from. The better the eval, the better the treatment.
